Patient Responsibilities

-You are responsible to provide us with accurate billing information at the time of service.

-If your insurance company requires you to choose a primary care provider (PCP). It is your responsibility prior to ensure that you have prior authorization.

-Your insurance company requires you to pay your copay at the time of each visit.

-If you do not have insurance coverage, you will be expected to pay at the time of your visit.

-It is your responsibility to understand any deductibles that may apply under your Insurance Policy

-Medical insurance does not always cover the entire cost of your medical care. If we believe a service we offer is not covered by your insurance coverage, we will tell you. In some instances, however we do not learn that a service is not covered until after we submit a bill. You are responsible for payment if your insurance company refuses to pay for a service.

-You are responsible for making sure we are participating with your insurance plan, failure to do so may require full payment

Question about your bill

What billing or insurance information will I receive?

Usually, after claim is processed by your insurance company, they will     send an explanation of payment/benefit to the patient/subscriber.

How long will it take to get things settled with the insurance company?

Usually, claims are processed through insurance company within six to eight weeks of receipt of the claim, unless there is an issue with the claim itself or with the patient's insurance coverage.

Why am I getting bills?

If the insurance has applied any amount to deductible, coinsurance, out of pocket expense, or if service is not covered under the patient's current benefit plan, then you would receive a bill, or insurance information needs to be updated(i.e. coordination of benefits)

I received a notice that my insurance company has paid on my bill - but I can't understand how they calculated their payment amount. Do you know?

You can contact your insurance company, as every insurance plan is different, or you can contact the office or billing service for explanation.
